Complete ESRD management — including dialysis, home therapies, and conservative care options — for patients throughout Minnesota and western Wisconsin.
Individualized ESRD Treatment Planning
Not every ESRD patient follows the same path. We help you understand all available options — including dialysis, transplant evaluation, home therapies, and conservative symptom management — to choose the approach that aligns with your health goals.
Personalized Dialysis Options
For patients who need dialysis, we explain the differences between in-center hemodialysis, home hemodialysis, and peritoneal dialysis, and help patients and families choose the option that best supports comfort, independence, and quality of life.
Coordinated, Compassionate Support
Managing ESRD can feel overwhelming. We provide ongoing monitoring, treatment adjustments, symptom support, and coordination with dialysis clinics, hospitals, and transplant teams to ensure seamless, connected care.
How We Support ESRD and Dialysis Care
End stage renal disease is the final stage of chronic kidney disease when the kidneys can no longer clear toxins from your blood. We help patients navigate their options in a clear, compassionate, and coordinated fashion.
Whether preparing for transplant, beginning dialysis, or choosing a symptom-focused (non-dialysis) approach, our team is here to support you.
Understanding ESRD and Treatment Options
Does every patient with ESRD need dialysis?
No. Dialysis is one treatment option for ESRD, but not all patients start dialysis right away — and some choose a non-dialysis approach focused on symptom control and quality of life. Your nephrologist reviews your labs, symptoms, and goals to help you choose the approach that best fits your health needs.
If I need dialysis, what options are available?
There are three main forms of dialysis:
In-center hemodialysis: Performed at a dialysis clinic several times per week.
Home hemodialysis: Dialysis at home with training and ongoing support.
Peritoneal dialysis: A home-based therapy using the abdominal lining to filter blood.
Your care team helps you understand which option aligns with your lifestyle, medical needs, and preferences.
How will ESRD or dialysis affect my daily life?
Advanced kidney disease brings changes, but many people continue to work, travel, and remain independent with the right plan. We help manage symptoms, energy levels, medications, and treatment schedules — and coordinate with your dialysis center to keep care as predictable as possible.
Support That Helps You Feel More in Control
